VAGINAL CYTOLOGY DURING PREGNANCY


Abstract
Vaginal cytologic study seems very helpful during pregnancy as long as its smear is correctly and strictly prepared. The data to be presented are based on our experience of 497 pregnant patients as well as other reports summarized.
1. The pregnancy pre-term type showed an accuracy of 86% ¡Æ in predicting that spontaneous delivery would not occur within the next 5 days.
2. The pregnancy tem type predicted that spontaneous delivery would occur within the next ;. 5 days with 85% accuracy.
3. Prolonged pregnancy is not always associated with post maturity, but it can occur at any time if placental insufficiency is present We have seen a post maturity in a prinigravida with an unknown duration.
4. A quantitative diminution or disappearance of cell clusters in spontaneous abortion is a most sensitive diagnostic or prognostic with accuracy over 90% as compared with a 71% accuracy based alone on the increasing karyopyknotic index.
5. Long term administration of progesterone is to be a known cause of congenital anomalies of the external genitalia of female fetus and also frequently results in missed abortion.
The misuse of, progesterone can be prevented by applying an adequate cytologic technique.
6. If estrogen test induces proliferation of the vaginal epithelium, it could be concluded as a poor prognostic pregnancy and one may assume that a relative deficiency of progestroncy substance exists and hormonal therapy is needed. On the other hand, if the vaginal smear is normal, hormonal treatment is not required.
